Vida Sarkodie, PMHNP  currently accepts Cigna, United HealthCare, Oxford, Optum at this time for this location.    Claims will be submitted to insurance for payment and patient will be responsible for portion as set forth by your insurance carrier/Non covered services.

We strongly recommend that you clarify ahead of time what type of coverage you have for mental health services (Psychiatry, Medication Management, Psychotherapy, and Group Therapy.  Ask about the deductible, percentage of coverage for each visit, maximum number of  visits covered yearly.

Copay is due at the time of each visit. Visits may be paid for by check, credit card, debit card or cash. Checks should be made payable to “Vida Sarkodie  Inc“.

We require24-hour notice of cancellation for all appointments. In the case of a bona fide emergency, exceptions will generally be made. When you do not keep an appointment and do not contact us with 24 hour notice it is a loss of appointment time that could be offered to another patient.
